MEDICAL INSURANCE

Team members will choose either a PPO or an HRA plan. The PPO plan offers in-network options at the FMOLHS level and PPO level and includes out of network benefits. The HRA is a consumer driven option with high deductibles, an employer-funded health reimbursement account and lower premiums.

Walk-in, daytime clinic for team members regardless of insurance coverage. $5 co-pays.

THE FRANCISCAN CLINIC

An annual health exam is covered at 100% for participating team members and spouses participating in the health plan.

DENTAL INSURANCE

Provides 100% coverage for preventative services. The plan includes coverage for restorative services and also orthodontia for dependents to age 19. Since there is no network, participants may seek services from any provider.

VISION INSURANCE

Full service plan with allowances for exams, frames, and contact lenses.

LONG TERM DISABILITY

Benefits provide 50% of monthly salary (to a max of $3,000 per month) after 90 days of continuous total disability. Employees may purchase a buy-up option of 60% of monthly salary (to a max of $10,000 per month).

When eligible: First of the month after six months of full-time status
Who pays: St. Elizabeth Physicians pays a percentage of the premium based on the team member's years of service


VOLUNTARY BENEFITS

FLEXIBILE SPENDING ACCOUNT
