Scheduling Appointments

Appointments may be scheduled by contacting our centralized scheduling desk at (225) 647-8511. We accept most insurance including Medicare and Medicaid, and while a referral is not required by all insurance carriers, you should check with your insurance provider especially when scheduling an appointment with a specialist.

Please note that you may also request a particular appointment time in MyChart, our Electronic Health Record Patient Portal. For more information, please see visit the MyChart page under the Patients & Visitors section.

Check-In Process

St. Elizabeth Physicians now utilizes an Electronic Health Record System. If you have not visited St. Elizabeth Physicians after June 2010, or if you are a new patient, you will need to complete a New Patient Information Sheet and a General Consent for Treatment . You will also given a copy of our Privace Policy. We will ask for a copy of your current insurance card. If you are an existing patient, we will ask you to verify your personal information and we will ask for a copy of your current insurance card.


Notice of Appointment Cancellation

We understand that sometimes things happen that interfere with your scheduled appointment time. When possible, we ask that you notify us 24 hours prior to your appointment. With prior notification, we are able to offer that appointment time to another patient in need.

If no prior notification is given and the appointment is missed, you may be assessed a fee. Please note that this fee will vary according to the provider’s specialty.
